If you’re 21 or older and have both Medicare and Medicaid, or receive long-term supports through Medicaid because you need help with everyday personal tasks, you’ll be covered by Community HealthChoices.
We welcome family-member caregivers through the PA Family Caregiver Support Program. Our caregivers are paid on a weekly basis and are offered a range of benefits as part of their full-time employment.

CHC will coordinate your health care coverage to improve the quality of your health care experience — serving more people in communities rather than in facilities, giving them the opportunity to work, spend more time with their families, and experience an overall better quality of life.

We emphasize finding the right caregiver for each patient by arranging as many meet and greets as needed.

Each patient is assigned a care coordinator who is responsible for all aspects of care and can be reached each day.
Coverage
We have an on-call team available after hours and weekends to deal with emergency coverage issues.
Focus on Quality
We have a skilled nurse who makes rounds, and trains CGs based on specific needs of individual patients.
